POLO
POLO (Purchase of Lifestyle Options)
POLO is a service for people with disabilities who either live locally, or are visiting Rockhampton, and who want to purchase services on a casual or temporary basis:

To discover and participate in events and activities

To enjoy a holiday away or attend specific social events

To maintain friendships and develop new relationships

To work towards the achievement of personal lifestyle goals
-
To participate in a variety of enjoyable recreation options, or simply have fun!
The target audience for this program does not necessarily need to have a formal diagnosis of disability.
Lifeworks
Lifeworks is an on-going community access service for individuals with disabilities who require support to access a variety of experiences, activities and relationships which are of significance to, and valued by the individual. Services are flexible around individual needs, but can broadly be described as almost any out of home activity except employment or vocational education.
Eligibility Criteria for Lifeworks:

People with a permanent disability

Aged between 5 and 65 years

Living in Rockhampton area
-
Have own individual funding package
If necessary, Access Recreation can assist an individual or his/her family to develop a funding application on their behalf and submit it to the relevant government department.
Leisure Lifestyles
Leisure Lifestyles is a time-limited recreation service, assisting individuals with disabilities to pursue an activity of their choice, within community settings, with their non-disabled peers. When people are able to pursue their activity without assistance from the worker, support is gradually withdrawn to the point of disengagement from direct service. Support can be provided for a maximum of two years.
Eligibility Criteria for Leisure Lifestyles:

People with a permanent disability, 5-65 years

Live in Rockhampton area

Will work with staff to achieve recreation goals
-
Not receiving similar services elsewhere
-
No foreseeable barriers to pursuing their chosen activity without support from Access Recreation
A waiting list exists for Leisure Lifestyle services.
Access For All
Access For All provides a watching brief on issues of physical access into and within public buildings, facilities and transport options. Local lobbying to improve access and participation by people with disabilities, and various other actions are taken at a systemic level. Support is given to actions taken by allied agencies.
Referral
People can self refer, or be referred by families, local service providers in the health and disability sectors, or any other party with an interest in the individual's quality of life. Referrals must have the person's consent.
